Medicaid Service Coordination

Rivershore, Inc. Medicaid Service Coordinator’s (MSC’s) are a group of highly dedicated and educated professionals who will work with you in achieving your life goals. We do this by:
• Assisting you in developing your Individualized Service Plan (ISP).
• Linking and referring you to requested supports and services that meet your specific needs.
• Ensuring both your Benefits and Entitlements
• Advocating with you to ensure your goals are being achieved
• Working in collaboration with community providers to provide a holistic approach

Our MSC’s are committed to:
• A person centered approach
• A strengths based plan
• Keeping you and your family up to date about related services
• Supporting you through times of transition
• Being knowledgeable about the field of developmental disabilities

Services that MSC’s provide:
• Working with you through the eligibility process to obtain services through New York State Office for People with Developmental Disabilities
• Enrolling you in Medicaid, an insurance program that opens access to many services
• Plan of Care Support Services
• Family Education Training for children 18 and younger
• Medicaid Service Coordination for children 3 and up through adulthood
• Applying for Supplemental Security Insurance (SSI) benefits
• Linking you to Medicaid Waiver services at the agency of your choosing
• Linking you to community supports and services

You may qualify for a Medicaid Service Coordinator if you have one or more of the following diagnosis:
• Intellectual Disability with an IQ below 70
• Autism
• Cerebral Palsy
• Neurological Impairment
• Epilepsy
People with a developmental disability are diagnosed prior to age 22 and it is expected that they will continue to have a developmental disability.

Rivershore, Inc. Medicaid Service Coordinators will meet with you throughout the year to find out how you are doing and ask you what you want for your life. Between meetings we will work at accessing those supports and services that you want. Rivershore, Inc. supports people with developmental disabilities as they pursue and achieve a meaningful life.
